Supportive care Dataset

Supportive care helps to prevent, control, or relieve complications and side effects of complex health conditions.

Sharing supportive care information improves communication between the different healthcare professionals. This ensures the patient receives coordinated care which reflects their wishes.

Our Supportive care Dataset provides healthcare professionals with the information they need to provide complex care for patients with chronic health conditions. Information is presented in an easy to understand format and helps care staff to monitor the treatment and symptoms of the patient.

What’s included?

Our Supportive Care Dataset provides a specialist view of information which is made up of specific coded entries. This is usually presented as a separate view alongside the Detailed Care Record.

This Dataset provides seven categories of information:

Advanced care planning

This section shows whether the patient has an end of life advanced care plan and advanced statement (Mental Capacity Act 2015). It also shows whether they have appointed a lasting power of attorney and instructions on whether CPR should be attempted.

Carers

This section summarises any formal carers or services involved in the care of the patient. It also shows the details of any informal carers and the next of kin.

Death details

This includes the place and cause of death.

End of life diagnosis

This includes any disabilities affecting the long term care of the patient.

End of life care status

This section shows whether the patients personal care plan has been completed and whether the palliative care plan needs to be reviewed. It also shows the patient’s medication, spoken languages, religion and whether they have been discharged from hospital.

Patient preferences

This section includes the preferred priorities of care, place of care and death. It also shows whether the patient is an organ donor.

Frailty

This section describes any general frailty and co-existing conditions which will have an impact on the care of the patient.
