Our Teleologic Dataset provides a bespoke view of patient information from the Teleologic Patient Content Store (PCS) system.
PCS helps thousands of clinical staff in cardiology, cancer care, diabetes, and many other clinical specialties to update patient records directly in clinic, ward or MDT meetings. This information includes discharge summaries and outpatient letters, inter-hospital transfers, patient pathway management and clinical audit reporting.
Our Dataset provides real time access to important patient information from the PCS system. This allows healthcare professionals from other settings to access real time acute data through the MIG. This includes GPs and community care providers.

It includes ten categories of tailored data from Teleologic Patient Content Store (PCS) system.
Demographics
This section includes the patient’s NHS number, age, gender and address. It also shows the name and practice of their current GP.
Summary
The summary view shows co-morbidities, allergies and medication recorded in the patient’s most recent discharge summary.
Problems
This view shows a list of any co-morbidities recorded in the patient’s most recent discharge summary.
Diagnosis
This view shows a past and present medical conditions.
Medication
This view shows a list of medication from the most recent discharge summary.
Allergies
This view shows a list of any allergies and adverse reactions from the most recent discharge summary.
Procedures
The procedures view shows details of any completed discharge summaries for the past three years.
Correspondence
This section shows a list of completed clinical letters for the past three years.
Forms
This shows the details of any forms which have been input for the patient over the last three years. It includes submitted, completed and pending forms.
Visits
This shows future and past hospital appointments. It includes a status column which shows if an appointment was cancelled or the patient didn’t attend.
