Our Specialist Datasets provide organisational feeds of patient information from a wide range of healthcare settings.

Healthcare professionals can use them plan complex care for patients who require long term or end of life care.

They provide specific patient information which can be used alongside data from the Detailed Care Record.

What’s included?

Chronic obstructive pulmonary disease (COPD) Dataset

This supports care planning for patients with COPD. It includes three categories of data:

  1. Diagnosis (home oxygen, vaccinations and smoking).
  2. Investigations.
  3. Monitoring.

Electronic palliative care coordination systems (EPaCCs) Dataset

This supports the coordination of an individual’s end of life care plan. It includes nine categories of data which can help to identify the needs, wishes and preferences of the patient. Information can be used to record care choices made by the patient.

  1. End of life care status.
  2. Social and demographic.
  3. Disabilities.
  4. Frailty.
  5. Carers.
  6. Advance care planning.
  7. Last days of life.
  8. Death details (date, place and cause).
  9. Express consent.

MIG end of life enhanced Dataset

This provides additional information to support end of life care planning for patients. It includes six categories of data:

  1. Advanced care planning.
  2. Carers.
  3. Death details.
  4. EOL diagnosis.
  5. EOL status.
  6. Patient preferences.

End of life ambulance Dataset

This provides end of life care information for ambulance trusts to use when responding to emergency calls. It includes the six categories of data, which are listed in the MIG end of life enhanced dataset detailed above.

Care plan Dataset

This includes eight categories of data which can be used to create a care plan for a patient.

  1. Consent to share data.
  2. Admission avoidance.
  3. Care planning.
  4. Advanced decisions (DNACPR, health and welfare, preferred place of care etc.)
  5. Other (residence, key safe, communication considerations etc.)
  6. Contacts.
  7. Examination and test results.
  8. Lifestyle.

Supportive care Dataset

This includes seven categories of additional information to support end of life care.

  1. Advanced care planning.
  2. Carers.
  3. Death details.
  4. EOL diagnosis (disabilities affecting care).
  5. EOL care status.
  6. Patient preferences (priority, place of death, organ donation etc.)
  7. Frailty.

Proactive care planning Dataset

This includes two categories of data to support pro-active care planning for patients.

  1. Proactive care planning (background, carers and support, professionals involved, special patient notes etc.)
  2. Recent medical observations.

Crisis care plan

This includes five types of specialist data, which can be used to create care plans for patients who require secondary mental health care.

  1. Common.
  2. Cancer.
  3. Heart failure.
  4. COPD.
  5. Dementia.

Medication and allergies Dataset

This provides a detailed view of a patient’s medication and allergies. It includes four categories of data:

  1. Risks and warnings.
  2. Current medication.
  3. Past medication.
  4. Medical issues.

EMIS Web community viewer

EMIS Web is a journal view of community data which is registered with a patient’s GP and other organisations involved in their care. This dataset provides a timeline of any medical entries made in the past 12 months.
